A review of endovascular treatment for medium vessel occlusion stroke

چکیده
Medium-vessel occlusions (MeVOs), that is, of the M2/3 middle cerebral artery, A2/3 anterior and P2/3 posterior artery segments, account for 25%–40% all acute ischemic stroke cases. Clinical outcomes MeVO with intravenous thrombolysis, which is current standard care, are moderate at best. With improving imaging technologies a growing literature, MeVOs increasingly recognized as target endovascular treatment (EVT). For time being, there limited but promising evidence safety efficacy EVT, many neurointerventionists already routinely offering EVT stroke, despite lack clear guideline recommendations. In this article, we review on summarize available literature strategies, commonly used selection criteria, techniques, outcome assessment stroke.

Endovascular Management of Stroke Patients with Large Vessel Occlusion and Minor Stroke Symptoms
Endovascular mechanical thrombectomy for stroke patients with large vessel occlusion (LVO) in the anterior circulation has become the standard of care based on several major randomized clinical trials. The successful result reported by these trials constitutes what may be the largest achievement in the history of neurological sciences. However, most of these mechanical thrombectomy trials (exce...

stroke patients with proximal occlusions of the main stems of cerebral arteries are no optimal candidates for i. v. thrombolysis. for many years interventional stroke treatment could not be established as alternative. this changed with the introduction of stent retrievers and flexible large lumen aspiration catheters. randomized trials now proved a significant benefit from intervention for a wi...
متن کاملDelayed stroke secondary to increasing mass effect after endovascular treatment of a giant aneurysm by parent vessel occlusion.
A 47-year-old woman, who had lost vision in her left eye because of a giant left supraclinoid internal carotid artery aneurysm, was referred for endovascular treatment. Parent-vessel occlusion was performed to obtain circulatory exclusion of the aneurysm. Eight days after treatment, she became hemiparetic and dysphasic. Repeat angiography showed compression of the left middle cerebral artery by...
متن کاملComputed Tomographic Perfusion Selection and Clinical Outcomes After Endovascular Therapy in Large Vessel Occlusion Stroke.
BACKGROUND AND PURPOSE Different imaging paradigms have been used to select patients for endovascular therapy in stroke. We sought to determine whether computed tomographic perfusion (CTP) selection improves endovascular therapy outcomes compared with noncontrast computed tomography alone. METHODS Review of a prospectively collected registry of anterior circulation stroke patients undergoing ...
متن کاملEndovascular treatment for ischemic strokes with large vessel occlusion: proven therapy and bright future.
I ntravenous tissue-type plasminogen activator (tPA) has been the mainstay and only therapy with proven clinical benefit in patients with acute ischemic stroke for the nearly 20 years. 1 Patients harboring a large vessel occlusions (LVOs) seemed to be recalcitrant to intravenous thrombolysis that portended a poor neurological recovery. 2 Catheter-based treatments offered a promise of higher rec...
